Thank you for your interest in the Pediatric Care Coordination: Community of Practice regional meetings! We’re excited to host these meetings and provide an opportunity for people who manage care for children and youth with special needs to share experiences, network, and collaborate at the regional level. The meetings will be a combination of small group discussion/networking and collective learning about innovative care coordination strategies in the region.

ACET will facilitate one meeting per region in either February or March and then will host a state-wide meeting in the early summer.

When you register below, please consult the image of Minnesota and register for the meeting for the region in which you primarily work.
