CDCS shared services

Definition

CDCS shared services: Services provided at the same time, by the same direct support worker, to CDCS participants who have entered into an agreement to share CDCS services.

Applicability

People on the following programs can use CDCS shared services:

  • · Alternative Care (AC) program
  • · Brain Injury (BI) Waiver
  • · Community Alternative Care (CAC) Waiver
  • · Community Access for Disability Inclusion (CADI) Waiver
  • · Developmental Disabilities (DD) Waiver
  • · Elderly Waiver (EW).
  • Covered services

    A person can only use shared services if:

  • · The services are in the personal assistance category (refer to CDCS – Personal assistance)
  • · The services are outlined in the person’s CDCS Community Support Plan
  • · The person enters into a written shared services agreement
  • · The people sharing services use the same financial management services (FMS) provider.
  • Non-covered services

    Shared services cannot be provided:

  • · To more than three CDCS participants by one worker at one time
  • · When more than one worker is providing services at the same time to people who are sharing personal assistance services
  • · For a child care program licensed under Chapter 245A or operated by a local school district or public school.
  • CDCS Community Support Plan

    When developing or amending the CDCS Community Support Plan, a person intending to use shared services must:

  • · Document the details about the shared services arrangement, including frequency, activity type and worker rates
  • · Describe how the shared services will meet the person’s needs and preferences
  • · Follow the CDCS process for lead agency approval of the plan (refer to CDCS – Process and procedure and CDCS Lead Agency Operations Manual, DHS-4270 [PDF])
  • · Describe the personal assistance services (i.e., activity type) the person will use while sharing services
  • · Describe the training needed for workers when providing shared services.
  • FMS provider requirement

    People sharing services must use the same financial management services (FMS) provider.

    Agreement

    To use shared services, a person must enter into a written shared services agreement using CDCS Shared Services Agreement, DHS-6633D.

    The agreement must include:

  • · The names of the people receiving shared services
  • · The responsibilities of the person/representative
  • · The name of the lead agency representative (i.e., case manager/care coordinator)
  • · The name of the FMS provider
  • · The shared services that will be provided
  • · The proposed schedule for the shared services
  • · The typical location where the shared services will be provided
  • · The person’s responsibility to add a description in the CDCS Community Support Plan of the training needed for workers who provide the shared services
  • · Instructions for all required documentation for the time and services provided
  • · Agreement by each person sharing services on the number of shared hours for the services provided
  • · Signatures of all parties involved in the shared services.
  • Back-up plan

    Each person must develop a back-up plan that accounts for service provision and billing in the absence of one of the people in a shared services setting due to illness or other circumstances.

    Discontinuation

    A person or representative may withdraw from participating in a shared services agreement at any time.
